Bringing Care Closer to Mothers and Newborns: Using the Gap Analysis Tool to Develop a Home to Hospital Continuum of Care.

By strengthening care at the household and community, and linking it to the peripheral and district facility levels, the household-to-hospital continuum of care (HHCC) approach ensures that mothers and newborns can access preventive services and in particular lifesaving care when they need it most – during the vulnerable days and weeks immediately following childbirth. The GAT also provides a way for program planners to systematically assess current mother and newborn (MNB) care, make a plan of action to address gaps in care, and ensure the involvement of essential stakeholders such as community members and health policy makers.
